Hunter Home Modifications is also proud to hold current Disability Service Standards(DSS) accreditation, the Disability Service Standards reflect contemporary practices that place people with disability at the centre of decision making and choice about their supports and services.

My Aged Care

On the 1st of July 2015 the Commonwealth Home Support Program (CHSP) replaced the ​Home and Community care program (HAAC).

The Commonwealth Home Support Program builds upon the previous HACC program and aims to deliver care and support to a large number of frail, older people to help them to be more independent at home and in the community, and to prevent their inappropriate or premature admission to residential care.

A key element of the CHSP is the focus on wellness, reablement and restorative care approaches in working with older people and their carers during assessment, planning and delivery of supports. It means building on older people’s strengths, capacity and goals to help them remain independent in their daily tasks and to live safely at home.

To access subsidised services provided by Hunter Home Modifications after the 1st July 2015 an assessment  will need to be completed by a Regional Assessment Service (RAS).

National Disability Insurance Scheme – Update

The first stage of the National Disability Insurance Scheme (NDIS) in New South Wales began in the Hunter area on 1 July 2013 for people with a disability who were living in the area on that date. There are two NDIS offices in the region, located in Charlestown and Newcastle.

Hunter Home Modifications has been engaged by the National Disability Insurance Agency to Project Manage and/or complete home modifications for participants of the scheme since the launch of the NDIS in the Hunter.
